Community Living Support and Education
REACH for Resources helps adults with disabilities achieve the highest level of self-reliance possible and integrate into their communities. We offer a wide range of support services that help our clients achieve their goals.
Individual Support
Our Community Living Specialists meet with clients one-on-one in their homes to provide the training and support they need to live as independently as possible. This service can be customized to meet individual needs and may include:
- Needs Assessment and Planning
- Housing Search
- Finance/Budget Management
- Meal Preparation
- Home Maintenance
- Time Management
- Employment Search and Job Skills Training
- Socialization Training
- Health, Hygiene, and Safety Training
- Regular Telephone Check Ins
- Crisis Support
- Advocacy and Referral
Individual Support Services are available through these options:
Reach Out - An affordable private pay program for families who lack eligibility for other public assistance programs. Scholarships are available based on financial need.
ORSILS, PSP, Waiver, or ARMHS - For clients who have been granted these Hennepin County supports.
