In Home Services

The In Home Services Department helps adults with developmental disabilities achieve the highest level of self-reliance, as well as, integration into their communities. We work with individuals by providing person-centered services that work best for each individual and their families. This type of support is different for each person and is based around their preferences, goals and abilities. We offer a wide range of support services including: housing search, employment search, employment skills, budgeting, bill paying, daily living skills, health, safety and socialization. After submitting the referral form, we will do an informational meeting with you to ensure this service is the right fit. If the service is a good fit, a Reach staff member* will be assigned. Then an intake meeting will be scheduled where you will receive the service agreement set, create outcomes and complete paperwork. * Due to staffing shortages you may be added to our Waitlist.

Waivered Services

  • The Developmental Disabilities (DD) Waiver (includes CDCS services) Provides funding for home- and community-based services for children and adults with developmental disabilities or related conditions.
  • Community Alternatives for Disabled Individuals (CADI) Waiver Provides many different services for independent living skills. Independent Living Services (ILS) help people to develop and maintain the skills necessary to live independently within the community.
  • Brain Injury (BI) Waiver Provides many different services for independent living skills. Independent Living Services (ILS) help people to develop and maintain the skills necessary to live independently within the community.

County-Funded Services

  • Developmental Disability Semi Independent Living Services (DD SILS) Individualized service that helps to teach and train people in many different areas. Some examples could include budgeting, preparing meals, shopping, and other skills that can help people maintain their independence.
  • Borderline Semi Independent Living Services (B SILS) Individualized service that helps to teach and train individuals with a borderline IQ in many different areas. Some examples could include budgeting, preparing meals, shopping, and other skills that can help people maintain their independence.

Community Contract Services

This is a service for individuals ages 16+ who do not meet the qualifications for waivered services; however while in school, they received special education or were on an IEP for a developmental disability. Reach is one of just a few organizations in the Twin Cities that contracts with Hennepin County to offer the Community Contract Program. This outcome-based program is designed so that once the desired outcomes are achieved, we would put together a graduation plan to end Community Contract services.
