Reach for Resources was awarded the MN DHS Own Home Grant to support individuals with disabilities who want to move out of congregate or provider-controlled settings and into their own homes. Through this grant, Reach Housing Navigators help people explore housing options, understand possible rent supports, and plan for the costs of independent living.

Congregate or Provider-Controlled Settings May Include:
- Assisted Living
- CRS, also known as group homes
- AFC, also known as Adult Foster Care
- Housing Support or Group Residential Housing
- ICS, also known as Integrated Community Supports or apartment programs

Reach Housing Navigators can help individuals and their teams look at income, rent, eligibility, possible subsidies, and payment assistance options. Housing Navigators help people compare options so they can make informed choices based on their goals, preferences, and needs.
